Steps to Complete a Payment Plan Modification
Follow the steps below to ensure your Payment Plan Modification Appointment can be completed without delay.
Complete the Payment Plan Questionnaire
Before scheduling your appointment, you must fully complete the questionnaire provided above.Gather Required Proof of Income
You must be prepared to provide all proof of income, including:
• The last four consecutive weeks of pay stubs
• Any proof of unemployment benefits
• Any proof of disability benefits
• Documentation of no income, if applicable
All income information must be available at the time of your appointment. If it is not, the appointment will need to be rescheduled.
Schedule Your Video Appointment
After completing the questionnaire and collecting all required documentation, you may schedule your video meeting using the link below.Log In Early to Your Appointment
You must be logged into the video meeting 10 minutes prior to your scheduled appointment time.
If you are late, the appointment must be rescheduled.Be Prepared for Verification Under Oath
During your appointment:
• You will be sworn in.
• You will be required to sign the Payment Plan Questionnaire and the Payment Agreement under oath.
• You must email the signed forms back to our office before the modification can be completed.
Please ensure you have access to your email during the meeting so the signed documents can be returned immediately.
